The following is the form and basic instructions for collecting signatures requesting a referendum to leave ITT crude oil underground indefinitely.
We can all download, print and start collecting signatures for this process.
BASIC INSTRUCTIONS FOR FILLING OUT THE FORM.
- Ideally the form should be printed on a sheet of bond paper. The form must be printed on one side and the note that follows as page 2 on the back.
- It must be filled out with a BLUE pen and printed letters.
- The ID and data of the signatories must be verified.
- If you make a mistake in the data in a box, that does NOT delete the form. Leave the box incomplete and move on to the next one.
- There can be no cross-outs or liquid paper repairs.
- The form also has a box for fingerprints. It is not mandatory to fill it out except in the case of disabled or illiterate people who cannot or do not know how to sign.
- The form must not be wrinkled, folded, stained or stapled. You must keep it as intact as possible.
Fill out as many forms as possible. If you live outside of Ecuador, you can collect signatures as well. Any Ecuadorian can sign as long as they are ELIGIBLE TO VOTE.
When collecting signatures, you are taking responsibility for the forms you fill out. You must sign the form below as the person responsible and attach a copy of your ID to the forms.
